19. Document how you contacted the Plaintiff or their attorney, along with the date and time of that contact.
20. Detail any responses you received from the Plaintiff or their attorney.
21. Complete the form by signing and dating where indicated.
22. Ensure that someone over 18 years old and not part of the action delivers a copy of the documents to the Plaintiff or their attorney.
23. Enter the address of the person who will deliver the documents.
24. Provide the name and address of the Plaintiff or their attorney.
25. Have the delivery person date and sign a receipt confirming the service.
26. Make two copies of all documents, ensuring one is delivered to the Plaintiff or their attorney.
27. Take the original and one copy to the Clerk’s office for filing, where the Clerk will process them.
