- Fill in the new requested extension date.
- Document how you contacted the Plaintiff or their attorney, along with the date and time of that contact.
- Detail any responses you received from the Plaintiff or their attorney.
- Complete the form by signing and dating where indicated.
- Ensure that someone over 18 years old and not part of the action delivers a copy of the documents to the Plaintiff or their attorney.
- Enter the address of the person who will deliver the documents.
- Provide the name and address of the Plaintiff or their attorney.
- Have the delivery person date and sign a receipt confirming the service.
- Make two copies of all documents, ensuring one is delivered to the Plaintiff or their attorney.
- Take the original and one copy to the Clerk’s office for filing, where the Clerk will process them.
