- Section 9 requires completion of concrete work details. Indicate if concrete work is excluded, or if it includes 2,000 cubic yards or more.
- In section 10, fill out the concrete subcontractor details, if relevant, similar to section 8.
- For section 11, provide information about the concrete safety manager if applicable.
