15. If applicable, include a No Objection Certificate from your employer if you are in government service or a public sector undertaking.
16. Declare any ongoing criminal proceedings or previous repatriation incidents.
17. For lost or damaged passports, provide details of the incident and the related local police report.
18. Register with the embassy or consulate if you have not done so, or indicate membership in any Indian organizations.
19. Complete the declaration section, affirming the truthfulness of the information provided and fulfilling any additional requirements for minors.
20. Review the completed form for accuracy, save the changes, and proceed to download, print, or share as necessary.
