- If applicable, include a No Objection Certificate from your employer if you are in government service or a public sector undertaking.
- Declare any ongoing criminal proceedings or previous repatriation incidents.
- For lost or damaged passports, provide details of the incident and the related local police report.
- Register with the embassy or consulate if you have not done so, or indicate membership in any Indian organizations.
- Complete the declaration section, affirming the truthfulness of the information provided and fulfilling any additional requirements for minors.
- Review the completed form for accuracy, save the changes, and proceed to download, print, or share as necessary.
