- Answer whether your application includes a J-2 dependent and indicate your INS alien registration number if applicable.
- Include the name and address of any attorney or organization representing you, if applicable. If you have an attorney, they must also sign the form.
- List all exchange visitor programs you participated in, starting with the first program, and provide explanations for any periods not covered by your official documents.
- Indicate whether your program received funds from U.S. Government sources, your own government, or an international organization.
