If you witness a person suddenly collapse for no apparent rea- son and that person is unresponsive and not breathing normally, you should treat this as a cardiac arrest.

Call 911. determine unresponsiveness. apply a tight splint.

A cardiac arrest is called unwitnessed if the patient is found without a pulse and no one was present at the time the patient collapsed. Most cardiac arrests in a critical care unit will be "witnessed" because the patient is usually connected to a heart monitor (or ECG).

In general, the accepted duration of a patient in ED—emergency department length of stay (EDLOS)—is 6 hours.

If there is no pulse, continue performing 30 compressions/2 breaths, checking for pulse after every 4 cycles until help arrives. Airway: tilt the head back and lift the neck to clear the airway.

In all adults and adolescents or in a witnessed collapse of a child or infant, you can leave the patient to get help before starting CPR if necessary. In an unwitnessed arrest of a child or infant, perform CPR for 2 minutes before leaving the patient to get an AED or to call emergency medical services.

If you see or hear someone suddenly collapse and they did not show any sign of choking, you don't have to worry about checking the airway. Assume it's a sudden cardiac arrest and follow the 3 Cs: Check for responsiveness (shake and shout), Call 911 and Compress at a rate of 100 per minute, about 2 inches deep.

Immediate general assessment and stabilization. Once the patient has arrived at the emergency department, and within 10 minutes of arrival, assess the vitals, providing oxygen if the patient is hypoxemic.
