If you are eligible, you may use form DS-82 and send in your renewal application by mail. If you are not eligible to send in the application by mail, you must use form DS-11 and apply in person.

Form DS-82 The DS-82 form or DS82 application for Passport Renewal is used when renewing a United States passport that is expired or near the expired date for no more than five (5) years beyond the expiration date on the passport.

Documents Required for Passport Renewal Original old Passport. Self-attested copies of the first two and last two pages of the passport. Self-attested copy of the Emigration Check Required (ECR)/Non-ECR page. Self-attested copy of the page of observation, if any, made by the Passport Issuing Authority.

You can fill out the form using the online form filler of the Department of State website (https://pptform.state.gov/) or print it out and fill it out by hand using a black pen. If you fill out the form electronically, at the end of the questionnaire there is an option to generate the form as a PDF file.

Renewal Application (DS-82) You should complete this form if you meet our requirements to renew your passport. Keep in mind that children under age 16 cannot renew their passports and must apply using Form DS-11.
