- Specify the priority of your request in the Priority I and Priority II sections, denoting your immediate needs effectively.
- Summarize the total number of books recommended in words in the designated field for clarity.
- After completing the form, provide your signature in the Signature of the Faculty field, along with your name and membership code.

- If applicable, obtain the signature and stamp from the Dean/Chairperson Special Centre to validate your requisition.
