Group I includes four (4) classes of surgical items that comprise a traditional “surgical count” performed by operating room (OR) staff: surgical sponges and surgical towels, sharps, small miscellaneous items (SMI), and instruments.

The count sheet is a safety measure used in healthcare organizations that list the name of the instruments/items in a surgical/procedural instrument tray and the number of each instrument/item contained in that tray.

Retained surgical items (RSIs), sometimes referred to as retained foreign bodies or retained foreign objects (RFOs), are any instruments, tools, devices, or surgical gauze unintentionally left in the body cavity at the completion of a surgery or other procedure, including vaginal sponges remaining after procedures or ...

Counting items, such as needles, compresses and instruments, in the operative field should include the review of fields, hampers and garbage men so that the count of items offered has no discrepancy with the number found at the end(2).

If the closing count is incorrect, the following steps are taken: Notify the surgeon immediately. A recount will be conducted. \ The surgeon will explore the wound.

All items should be completely separated during a count. Counts should be performed in the same sequence: sponges, sharps, miscellaneous items, and instruments. Items added during the procedure should be counted immediately and recorded on the white board and/or count sheet for retention in the patients' medical notes.

During the time-out, the entire operating room team reviews the patient's identity, the procedure, and the surgical site before surgical incision or the start of the procedure. The time-out is also a time designated for team members to voice any concerns about the patient's safety or the procedure.

Cavity count If a cavity within a cavity exists e.g. uterus, a count is completed prior to closure of this cavity. Final closing count On closure of the skin layer or when all countable items are no longer in use.
