To reduce the risk of dependence when treating acute pain, opioids should be used for five days or less. Opioid use disorder is a chronic brain disease that develops from repeated use of opioids. In its moderate to severe form, it is also known as addiction and can be life-threatening.

A signed “Start Talking” consent form must be maintained in the minor's medical record and the form must be separate from any other document the prescriber uses to obtain informed consent for other treatment provided to the minor.

14-day Prescriptions for Opioid Analgesics: The law generally prohibits a pharmacist, pharmacy intern, or terminal distributor from dispensing or selling an opioid analgesic pursuant to a prescription if the drug is to be used on an outpatient basis and more than 14 days have elapsed since the prescription was issued.

Most insurance companies allow a patient to get a 30-day supply about 5 days (give or take) early, but it is surprising how many patients think this means that they are allowed to use up the medication 5 days early.

No more than seven days of opiates can be prescribed for adults. No more than five days of opiates can be prescribed for minors. Health care providers can prescribe opiates in excess of the day supply limits only if they provide a specific reason in the patient's medical record.

7-Day Acute Limit: A new prescription for an acute condition is limited to a 7-day supply for a patient with no opioid prescriptions in the prior 90 days.

Like the 2016 guideline, the 2022 guideline recommends that when opioids are needed for acute pain, they should be prescribed at the lowest effective dose and for no longer than the expected duration of pain severe enough to warrant opioids.
