Follow the steps to successfully fill out the online application.

1. Click ‘Get Form’ button to access the application and open it in the online editor.
2. Complete the applicant information section. Fill in the student's first, middle, and last names, date of birth, gender, email address, home address, and phone number.
3. Provide the current school name and number, pupil ID number, and the current grade level of the student.
4. Enter the parent’s or guardian's full name, contact number, and email address. If there are multiple guardians, ensure all relevant details are included.
5. Indicate if the student has an Individualized Education Program (I.E.P.) or a 504 Plan, and specify if special testing accommodations are needed.

7. Select and rank the preferred Ingenuity schools by writing 1, 2, or 3 to indicate first, second, and third choices, recognizing the importance of ranking.
8. Review the Parent/Guardian permission statement, then obtain the signature and date from the parent or guardian.
9. Complete the Parent/Guardian Recommendation form, ensuring all sections are filled out and returned in a sealed envelope by the designated recommenders.
10. Distribute Teacher Recommendation forms to two teachers for completion and ensure they remain sealed until submitted with the application.
11. Once all forms are completed, compile all documents into one envelope for submission, ensuring that all documents are sealed and properly signed where required.
12. Submit the completed application before the deadline via mail or in-person delivery to the specified Ingenuity Project address.
