- Indicate if you were an independent contractor by selecting 'Yes' or 'No.'
- Complete your employment dates, including the last date of employment and the start date of employment in the format mm/dd/yyyy.
- Indicate how often you were paid by selecting one of the options: 'Weekly,' 'Bi-Weekly,' 'Semi-Monthly,' or 'Other.'
- Answer the questions regarding whether you quit or were fired, including the appropriate times if applicable.
- Continue to the back side of the form (as noted in previous instructions) to complete the payment details. Fill out the chart provided with the gross amount paid late for each pay period. Include the date due and the date paid, and if necessary, attach another sheet for additional information.
- Certify the statement regarding the accuracy of the wages due by signing and dating the form.
- If an authorized agent is filing on your behalf, ensure both you and the agent complete the necessary signatures and provide the agent's name.
