6. List the program you attended and include the dates of attendance; if you are unsure of the exact dates, approximate dates are acceptable.
7. Select the type of transcripts you are requesting: indicate how many official and unofficial copies you need.
8. If you would like to pick up your transcripts, select that option, or provide the mailing address for the institution or individual to whom the transcripts should be sent.
9. Review the Authorization Statement. By signing, you are verifying the accuracy of the information provided and authorizing the release of your transcripts.
10. Sign and date the form to complete your request.
11. Once the form is complete, submit it to Student Services via fax or mail as indicated in the instructions.
12. After submission, keep track of your request to ensure timely processing.
