- If applicable, provide details about your parents or legal guardians, including their occupations and Rotary membership status.
- Fill in the information regarding your chosen program type (Family to Family, Youth Camps, New Generations Exchange, etc.) and ensure that all questions are answered completely without shorthand responses.
- Attach or insert a recent passport-size photograph of yourself as per the instructions. Ensure this photo is included in all sets of your application.
- Submit the completed application in four sets. Original signatures are required in blue ink on all copies. If you used a typewriter, make good-quality photocopies for sets 2-4.
- After completing the forms, make sure to review all sections for accuracy and completeness. Use the checklist provided on the last page to confirm all parts are filled and necessary signatures are obtained.
- Save your changes, and if needed, download, print, or share the application form before submitting it to your local Rotary Club or District.
