Include the parent's or guardian's name, home phone number, school site, emergency phone number, and their relationship to the student.
- In the description of the incident section, specify the injury date, time, and place where the incident occurred. Document the person in charge at the time of the incident and describe the circumstances surrounding the injury. Include the extent of the injury and record if the student was taken to a doctor or hospital, along with the date, time, and hospital name.
- Detail any emergency care given and indicate by whom. Additionally, note the time parent notification occurred, the name of the doctor, who notified them, and the telephone number called.
- If there are any witnesses, provide their names, addresses, and phone numbers. Use an additional sheet if necessary to include all relevant witness information.
- Complete the signature information section by providing the name and signature of the person reporting the injury, the site coordinator, and the SFUSD site administrator. Include the dates for each signature.
- After completing all sections of the form, review for accuracy. You may then save your changes, download, print, or share the completed report as needed.
