- List any additional authorized partners, members, or officers, making sure they are also within DCU’s field of membership. Provide the required information for each individual.
- Complete the Certification and Agreements section. Enter your Tax Identification Number (TIN), certify under penalties of perjury, and agree to the terms and conditions outlined in the document.
- Lastly, ensure all required signatures are provided. Each individual listed in sections 3 and 4 must sign and date the application.
- Upon completion, review the application for accuracy. You can then save your changes, download, print, or share the completed form as needed.
