5. Proceed to Section 4. Indicate your preference for collection by checking the relevant box: either have it mailed to the address provided or request a phone call for collection from Monash Connect.
6. In Section 5, fill in your payment details. Authorize the total amount to be deducted from your credit or debit card, including the cardholder's name, card type, credit card number, and expiry date. Don't forget to include your signature.
7. Once you have completed all sections, review your application for accuracy. After ensuring all fields are filled correctly, save any changes, and prepare to download, print, or share your form as needed.
