- Complete Section I for all SAP requests. You will need to provide your Employee ID, Network ID (5 digits), Last Name, First Name, Middle Initial, Current Work Location (Campus #), Work Location Telephone #, and Position/Title. Indicate if you wish to delete a user from SAP by selecting 'YES' or 'NO'. If yes, enter the user's SAP ID.
- For new SAP User ID requests, complete Section II. Indicate if the new SAP User is replacing an existing one by selecting 'YES' or 'NO'. If 'YES', provide the previous User's Name, User ID, and Employee ID. Note that if 'NO', you must submit an approved SAP License Request Form alongside this request.
- Provide the date when the SAP user completed BEU/4.6 training. This training must be completed before issuance of the SAP User ID. Also, include the Composite Role Name. If necessary, run ZAU2 to find the correct Composite Role name.
- For any new or revised Composite Role or access change, complete Section III. Enter a similar existing Composite Role name if creating a new one, check the boxes in the Single Role column, and list any relevant Cost Centers. For changes, provide the existing Composite Role name and select whether you want to ADD or DELETE in the specified areas.
- To request a name or location change, complete Section IV and include your Previous and Current Names, respective Org. numbers, and work location details.
- Make sure to obtain required approvals by providing the Approver's printed name and signature. Also, acquire final approval from the Controller, Kenneth Huewitt.
- Once you have filled in all the sections as needed and have secured all necessary approvals, you can save, download, or print the form for submission.
