Follow the steps to complete the Mu1 Form online.

1. Click the ‘Get Form’ button to obtain the Mu1 Form and open it in your preferred document editor.
2. Log in to your company account. If you do not have an account, you will need to complete the Company Account Request Form.
3. Select the Filing tab from the main menu.
4. Choose Company (MU1) from the sub-menu.
5. Click the Create New Filing button to start a new form.
6. Select Add Activity to specify the business activities conducted by your company.
7. Choose the appropriate activities from the list provided and click Next.
8. Select the states where your company conducts these activities and click Next.
9. Associate the selected activities with the applicable states and click Next.
10. Review your selections and click the Request License button.
11. Select the licenses or registrations you need to apply for. If applicable, enter your existing license number; if not, leave that field blank and click Next.
12. Confirm your selected licenses or registrations and then click the Continue with Filing button.
13. Complete each section listed in the left navigation panel. You can either select each section individually or click the Next button to proceed to the next section.
14. An Individual (MU2) Form will automatically be created for each Control Person or Qualifying Individual identified. Each individual must establish their account and attest to the provided information.
15. After all individuals have attested to their forms and all sections are filled out, select Attest and Submit.
16. If all completeness checks are satisfied, attest to and submit the filing. Address any sections that require additional actions as directed.
17. Once submitted, ensure to mail the specific jurisdiction checklist, including the company name and NMLS ID, to the state within 5 business days.
