How to fill out the Mpera online
The Montana Public Employee Retirement Administration (Mpera) form is essential for managing your public employee retirement membership and beneficiary designations. This guide will provide you with clear instructions on how to effectively complete the form online, ensuring your information is accurate and up-to-date.
Follow the steps to fill out the Mpera form online
- Press the ‘Get Form’ button to access the form and open it in your preferred editing software.
- Begin by filling out the member information section. Include your last name, first name, middle initial, social security number, date of birth, employing agency, and gender.
- Provide your member’s mailing address, city, state, zip code, and daytime phone number, ensuring all contact information is current.
- In the primary and/or contingent beneficiary designation section, indicate your selections. Keep in mind that completing this section revokes all prior beneficiary designations.
- For each primary beneficiary, list their full name, gender, relationship to you, birth date, social security number, and the allocation percentage of benefits they will receive.
- If relevant, provide details for contingent beneficiaries, including their full name, gender, relationship, birth date, social security number, and allocation percentage. Use an additional line if needed.
- If you are designating a trust, charity, or estate, fill out the other designation section with the name, trustee/contact name, and address of the entity.
- Finally, ensure the required signatures are provided. This includes your member signature, witness name printed (not a beneficiary), and both the witness signature and date.
- After completing the form, make sure to save your changes. You can then download, print, or share the completed form as needed.
