Disability License Plates and Placards need to be renewed before the expiration date listed on the plate or placard. Disability Plates have a renewal registration fee of $101. Renewals can be completed online, by mail or by visiting an Illinois Secretary of State Office.

All applicants must fill out and sign the Application for Disabled Person Placard [dor.mo.gov] form 2769. If you are the applicant and apply in person at a Missouri Department of Revenue Office, the office can assist you in filling out the form.

Can the Temporary Disabled Placard be renewed? Yes, it may be renewed once, for a period not to exceed 180 days, by submitting a new Application for Disabled Person Placard (Form 2769), a new Physician's Statement for Disabled Person's Placard (Form 1776), and the $2 fee at any Missouri license office or by mail.

Renewals. The renewal notice has a section for your doctor to complete. We provide a fillable form. You can fill it out in your browser, print it, and take it to a Shelby County Clerk's Office.

A person with disabilities may obtain an application form from the license plate issuing official or download the form from the Motor Vehicle Division web page.

The permanent disabled person parking permit must be renewed every four years before the permit holder's birthday.
