9. In the felony convictions section, indicate whether the student has been convicted of a felony and, if applicable, provide details about the conviction.
10. Complete the parent or court-appointed custodian affidavit by initialing to verify the information and give consent for sharing the document. Sign and date the affidavit.
11. If necessary, arrange for a notary public to complete their section, which includes signature and expiration date.
