- If applicable, enter the special award number in the AWD. NO. box on the TRAY SLIP. For an open award, place a check mark in the OPEN box.
- Do not fill in the NO. OF ENTRIES line on the TRAY SLIP; this section is meant for judges to log the number of trays competing for the special award.
- Attach only the top section of the TRAY SLIP to your tray using masking tape, as transparent tape may not adhere well. Avoid taping the lower section to the tray.
- Once all sections are completed and reviewed, save changes, and you can then choose to download, print, or share the completed form.
