- In Part 4: Service Area, list the cities or counties where you will offer interpreter services as a permanent interpreter.
- Complete Part 5: Interpreter Certification. Sign and date the form to certify that you agree with all conditions set forth by the MDOS regarding your participation in the interpreter program.
