8. Enter the NEFT/IFSC code number of the bank as stated on the cheque issued by the bank. Remember to attach a photocopy of the cheque for verification.
9. Fill in the 9-digit code number of the bank and branch, also found on the MICR cheque, and ensure to attach a photocopy of the cheque for accuracy.
10. Read the declaration statement carefully and sign the form where indicated, specifying your name and providing the spouse's signature and name as well.
11. Complete the address and telephone number fields for both the pensioner and the spouse.
12. Once all fields are filled, you can save changes, download, print, or share the form as necessary.
