- Fill in your bank details, starting with the name of your bank, followed by the name and full address of the branch, including the telephone number.
- Input the account number as it appears on your cheque book in the specified field.
- Specify the account type by ticking the relevant box: Savings Bank Account (SB), Current Account, or Cash Credit Account.
- Enter the NEFT/IFSC code number of the bank as stated on the cheque issued by the bank. Remember to attach a photocopy of the cheque for verification.
- Fill in the 9-digit code number of the bank and branch, also found on the MICR cheque, and ensure to attach a photocopy of the cheque for accuracy.
- Read the declaration statement carefully and sign the form where indicated, specifying your name and providing the spouse's signature and name as well.
- Complete the address and telephone number fields for both the pensioner and the spouse.
- Once all fields are filled, you can save changes, download, print, or share the form as necessary.
