 - You will see a message showing your monthly download limit for Form-49. If you have remaining downloads, fill in the required information.
 - Click the 'Submit' button after completing the necessary fields to submit your request.
 - After submitting, a confirmation message will appear indicating that your request for Form-49 has been successfully submitted. You can now check your messages for the access password.
 - After downloading Form-49, your available forms count will decrease. If your limit reaches zero, contact the CTO office for further assistance.
 - Once you've completed all necessary downloads, remember to save any changes, or download, print, or share the form as needed.
