University Withdrawal Checklist: Communicate with your advisor and instructors immediately. Submit the University Withdrawal Agreement. If a student cannot appear in person to fill out a withdrawal form due to extenuating circumstances, he/she must send an email message from their CCP email account to counseling@ccp.edu and state the reason for withdrawal and include name, student ID number and phone number, and a counselor will respond.

No grade below a C in any course. This also includes P/F (pass/fail) and zero credit courses.

Student Responsibilities: Complete the change of major form. If you are only changing your degree, minor or emphasis, signature from your current academic department is required. Requests to change or add a major should be signed by an academic program representative of the new major. Return the form to Belmont Central.

A student who wants to withdraw from a course after the official Schedule Adjustment (Drop/Add) period and prior to the official start of final exams may do so by completing a Request for Course Withdrawal form.

Initiated in writing by completing an Administrative Withdrawal Request form. The request may be submitted anytime during the current academic term and no later than 30 days after the end of the current academic term.
