AGREEMENT INDIVIDUAL SELF-CERTIFICATION FORM. Tax regulationsi require the collection of certain information about each account holder's tax residency and citizenship status. This form is designed to capture your citizenship and residency for tax purposes.

Foreign Account Tax Compliance Act (“FATCA”) and Common Reporting Standard (“CRS”) regulations require financial institutions like us to collect and report information about where our customers are tax resident. Under these regulations, we have to ask you to provide the information requested in this form.

The form should be completed by the Account Holder the person who holds the Financial Account, regardless of whether such person is a flow-through Entity.

For a self-certification to be valid, however, it generally must contain the Account Holder's (i) name, (ii) address, (iii) jurisdiction(s) of residence for tax purposes, and (iv) tax identifying number for each Reportable Jurisdiction.

You should fill in the parts of the form that ask for your organisation's name, address etc. Put the same name and address as the bank uses to send you bank statements. You should sign and date the declaration at the end of the form. You can leave all the other parts of the form blank.
