- Include the unit description and quantity of units you wish to convert, making sure to follow the guidance regarding exercising the option for all units.
- If applicable, fill in the details of any locked-in units, including ISIN, reason for lock-in, and the release date.
- Review the declaration section to confirm that all provided information is accurate and that the units are not already dematerialized or subject to any liens.
- Obtain the required signatures from all holders in the signature section.
- If needed, complete the participant authorization section by entering the name and signature of the official, along with the participant's stamp and date.
- Before finalizing, double-check all fields for accuracy and completeness.
