NGB Forms 22 and 22A 3560For a copy of the NGB Form 22/22A, a request must be made to the National Guard Adjutant General's Office in the state from which the service member carried out their National Guard service.

NGB Form 22 will confirm your character of service at the time of your discharge, while the NGB Form 23 will verify your total time as “active duty.” In order to approve your application for this exemption, we must have both NGB Form 22 and Form 23.

Members of the National Guard are not required to complete NGB Form 22. But they will need to sign it when they're discharged. If you've lost your form (or you never received one), you'll need to contact the National Guard Adjutant's Office for your particular state.

Under the announced DoD policy, Guardsmen and Reservists will receive DD-214-1 forms, which will more accurately document their complete time of service and ensure they receive the military benefits they have earned.
