In order to get a disability rating and subsequent benefits for PTSD, veterans must complete VA Form 21-0781. This is a statement in support of a claim for service connection for PTSD. If your PTSD was the result of physical assault or military sexual trauma, you should use VA Form 21-0781a.

Use VA Form 21-0966 if you're still gathering information to support your claim, and want to start the filing process. Submitting an intent to file can secure the earliest possible effective date for any retroactive payments you may be eligible to receive.

When to use this form. Use VA Form 21-4142 to give us permission to obtain your personal information from a non-VA source like a private doctor or hospital. Examples of personal information may include your medical treatment, hospitalizations, psychotherapy, or outpatient care.

When to use this form. Use this VA form when claiming benefits for a Veteran's child who is at least 18 years old, but under 23, and attending school.

VA Form 21-10210 is used to provide information about an in-service injury or to give an update on the current severity of their service-connected conditions. However, VA form 21-4138 is no longer in use and the VA Form 21-10210 has been updated to replace it.
