10. Review and answer the questions in Section 6 regarding any adverse licensing history, and submit the 'Adverse Licensing Action History Form' if any answers are 'YES'.
11. In Section 7, answer the criminal history questions honestly, providing the 'Criminal History Form' if needed.
12. Complete Section 8 by listing any prior names used by the applicant.
13. In Section 9, provide information about prior experiences in the monument establishment or death care industry and ensure compliance with local zoning regulations.
14. Sign the application in Section 10, ensuring to declare that the information provided is true and correct.
15. Enter the applicant’s FEIN or Social Security number in Section 11.
16. When finished, save changes, and you may download or print the application for your records before submitting it via mail to the Division of Funeral, Cemetery & Consumer Services.
