- Identify your New Member Sponsor by entering their Last Name, First Name, and Middle Initial. Make sure to include the Sponsor's District Number and Home Club Number.
- Complete the membership fee and dues sections accurately, noting the New Member Fee, applicable sales tax, and total dues.
- Select your payment method, completing necessary details for check, credit card, or other payment types as required.
- Sign as the Applicant to confirm your agreement with the terms, ensuring you meet the age requirement.
- If applicable, have a Club Officer sign to certify that you are joining the Toastmasters club.
- Review your completed form for accuracy before saving your changes. You can then download, print, or share the form as needed.

To earn the DTM, you are required to: Complete two learning paths. Serve as a club officer for two six–month terms or one annual term, participate in the preparation of a Club Success Plan, and participate in a District-sponsored Club Officer Training. Serve a complete one-year term as a District leader.
