- In section E, indicate if your parent(s) had untaxed income from IRA distributions or pensions in 2012. If applicable, specify any amount that was rolled over.
- Proceed to section F. For students, indicate if you filed federal taxes for 2012. If yes, note if you need to request a tax return transcript. If no, list your earned income, and ensure to attach copies of all W2 forms.
- For parent(s), complete the same process as in step 7, indicating whether they filed federal taxes for 2012 and providing information on income as necessary.
- In section G, both the student and the parent must sign and date the worksheet to certify that all information is accurate. Ensure signatures are included.
- Finally, save the completed form. You can download, print, or share it as necessary, and ensure it is submitted to the UW-La Crosse Financial Aid Office via fax, mail, or email.
