How to fill out the Ccids online
The Child Care Information Data System (Ccids) provider portal is designed to assist eligible child care providers in completing necessary forms for agreement with the Ohio Department of Job and Family Services. This guide provides a detailed, step-by-step approach to filling out the Ccids online, ensuring a smooth experience for all users.
Follow the steps to successfully complete the Ccids online forms.
- Click ‘Get Form’ button to obtain the Ccids form and open it in the designated editor.
- On the login page, enter your 6-digit CCP User ID in the space provided.
- Enter your initial password, which varies based on your certification type. Follow the specific guidelines outlined for each type of provider.
- After logging in, you’ll be directed to the Home Page where your program information is displayed.
- In the Data Collection section, select each of the three required forms—Provider Agreement, Rate Form, and Banking Form. Complete all fields accurately.
- At the end of each form, select the checkbox to confirm your agreement with the given statements.
- Once all data is entered, select ‘Complete’ for each form, ensuring that all required fields are filled to avoid error messages.
- Submit all three completed forms by selecting ‘Submit’. This action sends the forms to ODJFS.
- After submission, confirm that you receive an email for your records and check back in the portal to ensure the status of each form is marked as ‘Submitted’.
- If needed, you can print or view each form post-submission, but remember to log out safely to secure your data.
