- Complete Schedule C if any amounts in the total grant request relate to qualifying equipment purchased by a pass-through entity where you held an ownership interest. List the pass-through entity’s name, FEIN, NAICS code, and ownership percentage.
- Fill out Schedule D by listing each Ohio county where the manufacturing facilities are located. Include details such as the street address and the principal product manufactured.
- Lastly, ensure all required signatures are added. If the taxpayer is an individual, their signature and date are necessary. For other entities, the signature of an officer or managing agent should be provided, along with the date.
