- Complete the Present Passport particulars section, detailing your nationality at the time of birth, current nationality, and relevant passport information, including passport number, place of issue, date of issue, and date of expiry.
- If applicable, indicate whether you hold dual nationality and provide details of the other nationality, including passport number, date of issue, place of issue, and date of expiry.
- If you have ever held a nationality other than your current one, detail the nationality, passport number, and relevant issuance information.
- Enter details related to the Indian passport of you, your parents, grandparents, or great-grandparents, including passport number and relevant dates.
- If applicable, respond to questions about your birth and residence in certain territories as of March 30, 1999, and provide evidence as required.
- Answer if you have ever been refused an Indian Passport or visa, including details of your application.
- Provide information regarding any previous PIO Card you may have held, including card number, date of issue, and place of issue.
- List the names and complete addresses of two individuals in India known to you, along with their contact information.
- Indicate the payment details for processing your application and provide the mode of payment along with the date.
- Review the completed form for accuracy, save your changes, and proceed to download, print, or share the form as needed.
