A Voluntary Declaration of Parentage (or paternity) (a VDOP) is a government form two parents can sign to create a legal parent-child relationship between a child and parent. It's often signed at the hospital when a child is born, but it can be signed later.

The Voluntary Declaration of Parentage (VDOP) is a form for a birth parent and other parent to voluntarily sign to legally establish parentage. It has the same force and effect as a judgment for parentage issued by a court. Establishing parentage may give your child the right to: Social security benefits.

Parents may obtain a Request for a Filed Declaration of Paternity form by contacting the California Department of Child Support Services at (866) 249-0773 or you may obtain additional information, including the form, by visiting California Child Support Services .

A person who has signed a voluntary declaration of parentage or paternity, or another interested party, may ask that the declaration be canceled (set aside) by filing a completed Request for Hearing and Application to Cancel (Set Aside) Voluntary Declaration of Parentage or Paternity (form FL-280).

A voluntary declaration of parentage or paternity is a California governmental form that, when signed by both parents, establishes them as the legal parents of the child. The form must be signed voluntarily. No one can force either person to sign the form.

Can I get another copy? Yes. Parents may obtain a Request for a Filed Declaration of Paternity form by contacting the California Department of Child Support Services at (866) 249-0773 or you may obtain additional information, including the form, by visiting California Child Support Services .

Parents may get a blank VDOP form from their local child support agency, local court (Family Law Facilitator), local registrar of births and deaths, and their local welfare office. Parents may also contact POP at askpop@dcss.ca.gov to request a blank form. The VDOP is not available electronically.

The Declaration of Paternity (CS 909) form is only to be used by unmarried couples to establish the paternal relationship with the child. You must use the official triplicate form (photocopies are not acceptable) that is available through local child support agencies.
