  19. If they are willing, confirm whether the joint declaration form has been submitted, ensuring it is filled out by your spouse's office.
  20. Respond to whether your children, if studying, are treated as dependents by marking YES or NO.
  21. Indicate whether your parents are dependent on you by marking YES or NO.
  22. Specify if your parents are living with you and include the month and year from which they have resided with you.
  23. State whether they are availing benefits from any other source by answering YES or NO.
