How to fill out the Superannuation Standard Choice Form online
Completing the Superannuation Standard Choice Form online is an essential step for employees and employers when dealing with superannuation fund choices. This guide offers step-by-step instructions to ensure a smooth and accurate submission process.
Follow the steps to successfully complete the Superannuation Standard Choice Form.
- Click the ‘Get Form’ button to obtain the Superannuation Standard Choice Form and open it in the editor.
- Begin by filling in Section A, where employees provide their details. Include your full name, employee identification number if applicable, and your tax file number (TFN). While providing your TFN is not mandatory, it is advisable as it can prevent higher tax rates on your contributions.
- In Section A, choose your preferred superannuation fund by placing an X in the appropriate box. Options include the APRA fund, a self-managed super fund (SMSF), or the employer-nominated fund.
- If you selected the APRA fund or retirement savings account, complete the fields for the fund ABN, name, address, contact number, and unique superannuation identifier (USI). Attach a letter from the fund confirming they accept contributions.
- For a self-managed super fund (SMSF), enter the required fund details such as ABN, fund name, address, and bank account information. Attach documentation verifying that it is an ATO-regulated SMSF, and confirm your role as the trustee or provide a letter from the trustee if applicable.
- Sign and date the form in Section A, ensuring you have attached the relevant documentation. Submit this completed section to your employer as soon as possible.
- Section B must be completed by the employer, including their business details, signature, and the nominated super fund's information if applicable.
- Once the employee returns the completed form, the employer should fill out Section C. This includes documenting the date the employee's choice was received and the date action was taken on the choice.
- Store the completed form for at least five years for record-keeping purposes. Do not send it to any external parties, including the Australian Taxation Office.
- Finally, review all entries for accuracy. Save changes, download, print, or share the form as needed for your records.
