- Indicate who is making the claim by checking either the Shipper or Receiver option. Then, provide the Account Holder’s name and Account Number.

- Record if the loss or damage was noted on the delivery record and whether it was reported to a SingPost Representative. Fill in the Representative’s Name if applicable, and indicate if Shipper’s Interest Insurance was purchased.
- Provide an itemized description of amounts claimed, attaching additional information as necessary. Make sure to include the Date, Value, and Amount of Claim for each item.
- Complete the form by printing your name and signing it as the Claimant. Specify the name of the individual to whom the cheque should be payable, ensuring it matches bank records.
- Finally, review all completed fields for accuracy. Save your changes, and then download, print, or share the Speedpost Form along with the required supporting documents.
