- Provide details of any previous conditions the patient may have had, including dates and durations, if applicable.
- Indicate whether the patient can be managed on an outpatient basis and provide a treatment plan detailing necessary tests and procedures.
- Estimate the length of stay and total costs associated with the admission, ensuring all values are clearly noted.
- Finally, ensure that the signature and stamp of the admitting physician or surgeon is added, including the name and specialty if the doctor is a medical officer.
