- For Option 2, if requesting a check or direct payment, ensure you have spoken with the Leadership Coordinator to obtain necessary approvals. This includes providing a vendor’s W-9, necessary lists, and invoices.
- Secure the signatures from the President or Treasurer and the Advisor in the respective sections before submitting your form.
