Abnormal monofilament testing has positive likelihood ratios >7 and negative likelihood ratios ranging from 0.07 to 0.61 for diagnosing neuropathy in patients with diabetes.

The 10 g Semmes–Weinstein monofilament evaluation (SWME) of 4 sites on each foot is recommended for distal symmetric polyneuropathy screening and diagnosis. A similar method has been proposed to diagnose 'high-risk' (for ulceration) feet, using 3 sites per foot.

Hold the filament perpendicular to the skin and use a smooth motion when testing. Use a 3 step sequence that includes (1) touch the skin, (2) bend the filament, and (3) lift from the skin. Do not use rapid movement.

Show the monofilament to the patient and touch it to his/her hand or arm so that he/she knows it does not hurt. Use the 10 gram monofilament to test sensation at the indicated sites on each foot as shown. Apply the monofilament along the perimeter of and NOT on an ulcer, callous, scar, or necrotic tissue.

Routine foot examinations, including the Semmes-Wenstein monofilament, should be performed on all diabetics to detect possible PN (ADA, 2012). Any healthcare provider, including nurses, can perform this test.

A monofilament test is done to test for nerve damage (peripheral neuropathy), which may be caused by conditions such as diabetes. The monofilament is a small strand of nylon attached to a plastic base. The provider uses this monofilament to check for loss of feeling on your foot.

The normal monofilament was 2 g (Filament Index Number (FIN) 4·31) for the radial cutaneous and 4 g (FIN 4·56) for the sural nerve, although heavy manual laborers demonstrated a threshold of 10 g (FIN 5·07) for the sural nerve.
