Filling out the Narf Form is a straightforward process that helps facilitate orthodontic care for patients located outside the continental United States (OCONUS). This guide provides clear instructions to ensure users can complete the form accurately and efficiently.
Follow the steps to complete the Narf Form online
- Click ‘Get Form’ button to obtain the form and open it in your preferred editor.
- Enter the patient's name in the designated fields, including last name, first name, and middle initial as per the uniformed services ID card.
- Provide the patient's date of birth by entering the month, day, and year.
- Indicate the patient's sex by selecting the appropriate checkbox.
- Select the relationship of the patient to the sponsor by checking the relevant box.
- Input the sponsor's name similarly as you did for the patient, including last name, first name, and middle initial.
- Enter the sponsor's Social Security number or Department of Defense Benefits Number accurately.
- Fill in the patient's complete mailing address, ensuring all details are included.
- Provide phone numbers for both the sponsor and patient, including the area code or country code.
- State the referring overseas dental treatment facility or TRICARE Area Office, including the name and location.
- Check the primary reason for referral by selecting the appropriate option.
- Describe the orthodontic service being referred, including any applicable CDT codes.
- Submit details regarding command sponsorship and time on station in OCONUS.
- Add any relevant remarks or additional information that may support the referral.
- Type or print the name and title of the person completing the referral form.
- Sign and date the approval section of the form.
- Ensure the sponsor or family member certifies their enrollment in the TDP and signs accordingly.
- If necessary, the referring party must confirm the veracity of the information by initialing and dating the form.
- Finally, input the tracking number for use by the overseas dental treatment facility or TRICARE Area Office.
