15. Type or print the name and title of the person completing the referral form.
16. Sign and date the approval section of the form.
17. Ensure the sponsor or family member certifies their enrollment in the TDP and signs accordingly.
18. If necessary, the referring party must confirm the veracity of the information by initialing and dating the form.
19. Finally, input the tracking number for use by the overseas dental treatment facility or TRICARE Area Office.
