The original Form 1188 is to be sent to HRSSC. If you send NAPS HQ your original Form 1188 package, NAPS HQ will keep its respective copy and forward, as a courtesy, the remaining copies to USPS HRSSC Benefits Office at PO Box 970400, Greensboro, NC 27497-0400.
