- If the recipient is a CSUSB student, ensure the CSUSB Financial Aid Office is notified in writing and attach the notification to your request.
- Attach a W-9 form if this is the first stipend payment for the recipient.
- Certify that the expenditure is aimed at supporting the instructional and service activities of CSUSB.
- Provide the name and contact information of the person preparing the Stipend Request Form.
- The recipient must sign and date the form.
- Obtain the signature and date from an authorized signer on the account for approval.
- Once completed, save any changes, and download, print, or share the form for submission.
