The IRS Form 9143 is basically their way of saying they need a proper signature to process your return. Form 9143 is a request for you to provide information that was missing from your tax return (it's most commonly used when a taxpayer fails to sign the return). Include a copy of the letter you received. A separate ETA-9143 with all relevant attachments is required for each YouthBuild work site.
