- In Part F, check the boxes to confirm you have read the explanatory notes and declare that all information provided is accurate. Sign, date, and provide your name and NRIC No.
- Skip Part G if you are not the employer. Otherwise, allow the employer to complete their section about acknowledging the childcare leave. They should sign and date this section as well.
- Once all parts are completed, review the form for accuracy. You can then save changes, download a copy, print, or share the completed form as needed.
