10. After thoroughly reading the ONRR Acceptable Use Policy, check the box to indicate your agreement. Print the completed form, sign it, and have your supervisor sign and date it as well.
11. Ensure that all required fields outlined in red are completed. Mail the original signed form to the specified address, avoiding overnight delivery.
12. Upon mailing, please wait for confirmation of the submitted information. The processing usually takes two to three weeks.
