10. Provide at least two character references who are not related to you. Include their names and addresses.
11. If you have family members or relatives who are members of PHCCI, list at least two of them along with their names, relationships, and addresses.
12. Indicate whether you are a member of another cooperative. If so, provide names, addresses, and positions of your membership.
